La novela narra la historia de Stefan Trzyniecki, alter ego de Lem, un joven doctor que, en los primeros meses de la invasión de Polonia, encuentra empleo en un hospital psiquiátrico enclavado en un bosque remoto. La locura del exterior se filtra poco a poco entre los muros del hospital, y así Trzyniecki se empeña en salvar a sus pacientes en ese lugar que parece «fuera del mundo», frente a un grupo de sádicos doctores que realizan atroces experimentos con los enfermos internados en el centro.
