Is that annoying squeal normal when you first turn them on and is that background shush normal?

Yes, the squeal is normal when you first touch the device to put it in your ear but after that, it is fine. There is no background noise after that unless one has tinnitus.

Is this an OTC hearing aid or PSAP?

In my opinion, this is a personal sound amplifier and works great for watching TV or in and around the house. The device picks up every road noise when worn to drive to the store even on the lowest level settings.

The Audien ATOM Rechargeable Hearing Amplifier is a nearly invisible hearing aid designed for those in need of assistance with hearing. The removable antenna is clear in color and very discreet when the hearing aids are inserted. The charger comes with a wall transformer that plugs into a standard 120-volt outlet. The package comes with one pair of hearing aids, with one size earbud but three sizes of silicone cups. Customers who have small ear canals have rated them highly. Some customers found them difficult to place in their ear, and the on/off switch and volume control were hard to do. There is an annoying squeal when the device is first touched to put it in the ear but after that, it works fine.
