If you can't find the right size, I have seen therapists make their tools using various size beads on a string and holding the string tight in the patient's mouth. There are YouTube videos that show the techniques.

However, they are not suitable for use with autoclaving. Some children might find that the size of the tool is either too small or too large. The tool requires good tongue control, which might make it difficult for some people to use. It is a well-designed product, but its quality is debatable for some people.
